Semaglutide has helped many people lose a meaningful amount of weight. What it cannot do is decide where that weight comes off, or tighten the skin it leaves behind. That is where body contouring comes in. Because Bellava MedAesthetics & Plastic Surgery Center in Bedford Hills offers both a physician-supervised semaglutide weight-loss program and non-surgical and surgical body contouring, we are often asked how the two fit together.
What does semaglutide do that body contouring doesn’t, and vice versa?
Semaglutide reduces total body weight; body contouring reshapes specific areas. They answer different questions, which is why they work well as parts of one plan.
Semaglutide is a GLP-1 receptor agonist, the same active ingredient found in Ozempic® and Wegovy®. It reduces appetite and helps you feel full sooner, so you eat less. In the STEP 1 trial, adults taking semaglutide 2.4 mg weekly alongside lifestyle changes lost about 15% of their body weight on average over 68 weeks. That loss happens throughout the body, in a pattern set largely by genetics.
Body contouring, by contrast, does not meaningfully change your weight. It targets particular areas: a stubborn pocket of fat that diet will not shift, or excess skin and separated abdominal muscles that no amount of weight loss can fix.
Can CoolSculpting help after semaglutide?
Yes, for the right problem: stubborn, pinchable pockets of fat that remain once your weight has settled.
CoolSculpting uses controlled cooling (cryolipolysis) to target fat cells in a specific area, such as the abdomen, flanks, thighs, upper arms or under the chin. The body then clears those cells gradually over the following weeks to months. It is non-surgical, needs no incisions and typically involves little or no downtime.
CoolSculpting is not a weight-loss treatment, and it does not tighten loose skin. It is a finishing tool. Many people find that after losing weight with medication, one or two areas still do not match the rest of their body. Those are the areas CoolSculpting is designed for.
When is a tummy tuck the better option?
When the main issue is loose skin or separated abdominal muscles rather than fat. Significant weight loss, like pregnancy, can stretch the abdominal skin beyond its ability to spring back.
A tummy tuck (abdominoplasty) removes excess skin and fat from the abdomen and can repair separated muscles (diastasis recti). At Bellava, tummy tucks are performed by board-certified plastic surgeon Dr. Chang Soo Kim. Depending on how much skin is involved, the options range from a mini tummy tuck for the lower abdomen to a full or extended tummy tuck for larger amounts of skin around the abdomen and sides.
A tummy tuck is not a weight-loss procedure. As our tummy tuck page explains, the best candidates are at or near their goal weight and committed to maintaining it.
Why does rapid weight loss leave loose skin?
Skin stretches gradually as weight is gained, and it does not always shrink back at the same pace when weight comes off, especially when the loss is large or fast.
Age, genetics, sun damage, smoking history and how long the skin was stretched all affect how much it recovers. Faster weight loss gives the skin less time to adapt. This is part of what people mean when they talk about “Ozempic face” or “Ozempic body”: not a side effect of the medicine itself, but the visible result of losing fat volume quickly.
For some people, the skin firms up well over time. For others, especially after losing a large amount of weight, the remaining skin is something only surgery can remove.
In what order should I do semaglutide and body contouring?
In general: lose the weight first, let it stabilize, then contour.
- Weight loss phase. Work with your provider on a semaglutide plan, with gradual dose increases and regular check-ins.
- Stabilization. Once you are at or near your goal and your weight has been steady, it becomes clear what remains: stubborn fat, loose skin, or both.
- Contouring. Non-surgical treatments such as CoolSculpting address localized fat. A surgical consultation addresses loose skin and muscle separation.
Contouring before your weight is stable can mean treating an area that would have changed anyway, or, with surgery, a result that shifts if you lose more weight afterward.
Do I need to stop semaglutide before a tummy tuck?
Tell your surgeon and anesthesia team that you take semaglutide, well before your surgery date. They will tell you whether and when to pause it.
GLP-1 medicines slow stomach emptying, and anesthesia safety depends on an empty stomach. Professional anesthesia guidance on GLP-1 medicines before surgery has been updated more than once, so instructions are individual and come from your own surgical team, not from a general article. Do not stop or restart a prescribed medication on your own.
Can semaglutide and body contouring be part of one plan at Bellava?
Yes. Our semaglutide service page notes that the program can be combined with complementary treatments such as body contouring and skin tightening. Having the medical weight-loss program, med spa treatments and a plastic surgeon under one roof in Bedford Hills means your plan can be coordinated rather than pieced together across several offices.
Frequently asked questions
Will CoolSculpting tighten loose skin after weight loss?
No. CoolSculpting reduces fat in a targeted area. Loose skin is usually addressed surgically, for example with a tummy tuck, depending on the amount and location.
How long should my weight be stable before a tummy tuck?
There is no single number; your surgeon will assess this at consultation. The general principle is that you should be at or near your goal weight and able to maintain it.
Can I keep taking semaglutide after body contouring?
That is a decision for you and your prescribing provider. Many people use a maintenance plan after reaching their goal, and keeping your weight stable helps protect contouring results.
Is it normal to have “Ozempic face” after weight loss?
Losing fat quickly can make the face look thinner or older, because facial fat contributes to fullness. It is a result of weight loss, not a direct effect of the medicine. Discuss any concerns with your provider.
Does a tummy tuck remove a lot of weight?
No. A tummy tuck removes excess skin and some fat from the abdomen and can repair muscles, but it is a contouring procedure, not a weight-loss one.
